a) To maintain contact with participants who decide to attend the conference. b) For marketing purposes.
Forms
The website includes a form titled “Konferencja 2019.” By choosing to complete and submit the form, you accept the terms and conditions (TERMS & CONDITIONS). Data submitted via the form is encrypted and accessible only to authorized personnel.
Google Analytics
We collect anonymous data for website traffic analysis, including:

- Website traffic source These data are collected through the web application Google Analytics.
Data Security

Conference Terms & Conditions

These Terms & Conditions apply to the CRANIA Conference: “Consensus in Diagnosis and Physiotherapy of Temporomandibular Joints” organized by Fizjoterapia i Klinika Stomatognatyczna Dr Małgorzata Kulesa-Mrowiecka.
Participation
Purchasing access to the conference/workshops/courses is equivalent to accepting these terms. Participation is verified by name lists at the conference/workshop/course reception desk.
Validity
The purchased access is valid only for the date specified on the invoice.
Right of Withdrawal
Service withdrawal without providing a reason is possible within 14 days in accordance with the Polish Consumer Rights Act of May 30, 2014. Withdrawal must be submitted online. The purchase cost will be refunded upon receipt of the signed corrective invoice.
Recording and Photography
Recording or photographing the course/conference requires the explicit consent of the presenter.
Definitions
- Terms & Conditions: This document outlining rules for participating in events organized by the Clinic.
- Website: Any conference/workshop/course website managed by the Organizer (e.g., www.konferencjacrania.pl or www.kursycmd.pl).
- Organizer: Fizjoterapia i Klinika Stomatognatyczna Dr Małgorzata Kulesa-Mrowiecka, Basztowa 3, 31-143 Kraków, NIP 677 208 55 25.
- Participant: Any individual using the Website to register for an event.
After payment through the Website or bank transfer, an invoice (“FAKTURA”) will be sent via email.
General Provisions
These terms outline the rules for purchasing participation in events via the Website. Users are obligated to read and accept these terms before completing their registration.
Registration Process
To register, users must:
- Fill out the registration form.
- Pay the full participation fee.
Registration is confirmed upon receipt of payment. Discount eligibility is determined by the payment date, not the registration date.
Order Confirmation
The Organizer reserves the right to determine the opening and closing of registration. Participants will receive confirmation of their registration by email within 10 minutes of submission, including payment instructions.
Failure to provide necessary data may result in service denial.
Personal Data Processing
In accordance with Article 13 of Regulation (EU) 2016/679 (GDPR):
- The data controller is Fizjoterapia i Klinika Stomatognatyczna Dr Małgorzata Kulesa-Mrowiecka.
- Data is processed for purposes related to event participation.

Payment
Payments may be made:
- Directly through the Payment Page following registration;
- Via bank transfer using the details provided in the confirmation email.
Payment must be made in full within 35 days before the event.
Course/Conference Delivery
Participants will receive an invoice:
- Via email approximately 10–20 minutes after payment;
- Within 15 days of the following month after payment confirmation.
Access to the event is granted upon check-in at the reception desk.
Organizer’s Responsibility
The Organizer agrees to:
- Deliver the event with due care and professional standards;
- Ensure proper content delivery as per the description;
- Take responsibility only for their own materials.
The Organizer is not liable for third-party content.
Registration Rules
Information about the courses is distributed through industry press, websites, and mailing lists. Registration is possible by phone or online via the Website.
If demand exceeds capacity, a waitlist is created based on payment order. Participants will be notified as spots become available.
Participant Rights & Responsibilities
- Courses are open to all unless specified otherwise.
- Registration and payment are required for participation.
- Participants must register at the reception before the event.
- Participants will receive course materials on-site.
- Seating is unassigned.
- Certificates are issued after the event.
Refund Policy
Refunds are granted if:
- The event is canceled;
- A resignation is submitted at least one month prior to the event (with a 200 PLN deduction).
No refund is available if:
- The participant does not attend;
- The resignation is submitted more than 14 days after payment unless justified by a documented emergency.
Refunds are processed within 30 days via bank transfer or email notification of denial.
Complaints Procedure
All inquiries, comments, or complaints should be sent via email to drkulesa@gmail.com.
Complaints must be submitted within 14 days of the incident. They should include full name, address, and details of the issue.
Complaints are reviewed within 14 days. Decisions are final and communicated via email.
Final Provisions
- The Organizer is not responsible for incorrect email addresses.
- The Organizer is not liable for lost or misconfigured email/data due to user settings.
- These terms are available on the Website.
- The Organizer may modify the terms at any time by publishing a new version online.
- The Organizer reserves the right to:
- Modify Website content at any time;
- Temporarily suspend access to the Website;
- Refuse liability for damages caused by site malfunction or unauthorized data access.
In matters not regulated by this document, Polish civil law shall apply.
These Terms & Conditions are effective as of June 30, 2018.
